My first attempt to produce a better usual-suspects submission mechanism was a bit of a disaster; a friend quickly found some nasty problems with the script. Looking back I think that concentrating on one particular reliability constraint resulted in missing the wood for the trees.
Anyway once I'd got that sorted I went to test the fixed version. I'd already set up a dummy list to test with ... and accidentally typed the runes to use the real version instead. So 80-odd people got a crappy test message.
Finally when I announced the new submission mechanism to the people who get to use it, I included a relevant typo in the instructions, and had to send a correction immediately afterwards.
