Add the powder to a very small volume of milk; stir; and when the paste is smooth add the rest of the milk. Then microwave.

I used to do that, but later tried adding the powder half way through, and didn't find the result appreciably different, and it's much less effort.

If you use drinking-chocolate powder, yes. If you use cocoa it goes lumpy. I hypothesize the difference is due to the former holding the cocoa grains apart with soluble sugary stuff.
